To register for the Forum:

(1) Return the completed form to the Forum by clicking the
submit button at the bottom of the form. The transmission of the information will be secure, and encrypted.

If you submit the form by clicking the submit button, you will see a statement that says: "This document has accessed more than one site and might be trying to share data among different locations. This could be a privacy concern. Are you sure you
want to continue."

You may be assured that there is no privacy concern. Only the Forum and the secure, encrypted mail server it uses are the only sites accessing this information. Your information is secure, and
encrypted.
